So far it looks like the 18+ "adult" games are games where there is female characters almost completely naked or completely naked. A few of them have a bit of blood here and there but not ludicrous gibs. The reason I think this is true is because sex sells. We know it sells. Having a female character you will never see in game show a lot of cleavage in a trailer or artwork or the game's cover is a nice way to try to attract more people to play the game. It looks like the more skin you can show the more interesting the game might be. This might be true for some games where eye candy is the only thing that matters because gameplay sucks. At least they will have something nice to look at.

I think that labeling a game 18+ allows players to believe that they are in fact playing a game with mature adults away from all the annoying teenagers and their trolling. As far as I can tell, this is false. Many parents will allow their kids to use their credit cards for MMOs because the price is low compared to some of the other hobbies out there. While they are fewer in number, it does not really make much of a difference. There are always enough players that are being assholes to annoy you. Anyone is capable of being a complete jerkass online if they want to. Their age does not matter. In fact, I feel like college people are more likely to be assholes than those under 18.

As far as I can tell, when people picture an adult MMO, they picture mature intelligent players who do not squabble over small issues and are rational people who are willing to discuss to solve issues. Trolls are believed not to exist and players do not scream at GMs. This is just some idealistic paradise that people are trying to strive for in an MMO. The problem is that we want something like this, but we know someone will always be there to ruin paradise.
